INTRAMUROS, Manila—The management of the Bureau of Immigration (BI) exhorted its officers assigned at the Ninoy Aquino International Airport (NAIA) and other ports to keep up their vigilance after it was announced that the Philippines retained its Tier 1 ranking in the US government’s human trafficking index for 2019. “I commend and congratulate the men and women of our port operations division (POD) who guard our ports day and night to ensure that countrymen are saved from the scourge of human trafficking,” Immigration Commissioner Jaime Morente said in a statement. The Philippines retained for the fourth consecutive year its Tier 1 ranking in the recently-released 2019 Trafficking in Persons Report of the US Department of State. Morente said that the rating is “a tribute to your hard work and dedication to your duties as gatekeepers of our country.”

Morente urged Bureau of Immigration personnel in the different ports to “remain vigilant as there is no sign that these human trafficking syndicates who prey on the poor, including minors, will stop their nefarious activities.” Bureau of Immigration Port Operations Division Chief Grifton Medina, for his part, said “the latest TIP report will surely inspire and motivate POD personnel manning the ports to sustain their efforts in fighting human trafficking.” Medina said there will be no letup in the bureau’s anti-trafficking campaign even as he reported that more than 10,000 travelers were stopped from leaving the country from January to April this year, mostly for having incomplete travel papers and questionable purpose in going abroad. A Tier 1 rating means that the Philippine government fully meets the minimum standards for the elimination of trafficking and has continually demonstrated serious and sustained efforts to combat human trafficking. The Philippines leads the ranking in East Asia and the Pacific, sharing the Tier 1 status with Japan, South Korea, and Taiwan. Other countries mentioned in the report who were given a Tier 1 rating are Australia, Canada, France, Netherlands, New Zealand and Spain. "The Tier 1 status was given to only 33 countries all over the world," said Morente. "We are proud to be a big part in the government's efforts against human trafficking. We will not stop protecting our fellow Filipinos from this threat. The result of this report is a reminder why we do what we do," he added. The report also acknowledged the BI’s accomplishment in deferring the departure of 24,753 passengers due to incomplete or missing travel documents or misrepresentation, referring for investigation 286 potential cases of suspected trafficking, identifying 286 potential victims of trafficking, and arresting nine suspected traffickers. BI stopped 199 foreign registered sex offenders from entering the country last year, the same report stated.

LT GARY DALE GIMOTEA PCG bagged bronze medal in minus 73kg division. Members of the AFP and PNP participated in the tournament.

SW2 Karen Ann Solomon PCG, SW2 Ruth Dugaduga PCG and SW2 Helen Dawa PCG defended their title when they bagged the medals in three categories during the national judo tournament held at SM City Marikina last May 2009. These achievements earn them a slot in the 25th South East Asian Games (SEA Games) to be held in Laos in December 2009.

Also, during the 1st Asian Matial Arts Games held in Bangkok, Thailand in August 2009 where SW2 Karen Ann Solomon PCG and Helen E Dawa PCG bagged  two bronze medals. More than 41 countries participated in said tournament.

In May 2009, coast guard athletes who are members of he Philippine dragon boat national team garnered eight gold medals and three silver medals in the Macau International dragon Boat races, and in the 6th nanning international dragon boat invitational tournament in china. Likewise, team successfully defended their title as world dragon boat champion and record holder in the 200-meter category in competitions held in Prague, Czech Republic between 26 and 30 August 2009.

On top of this, the Philippine coast guard is active in promoting marine environmental protection by regularly participating in fun runs advocating environmental awareness, such as the “DZMM Takbo sa Kalikasan” and Earth day marathon.

CEBU, Philippines—Bureau of Immigration (BI) officers at the Mactan-Cebu International Airport (MCIA) rescued 17 suspected human trafficking victims who were illegally recruited to work as household service workers (HSW) in the Middle East. Bureau of Immigration Port Operations Division Chief Grifton Medina said the female passengers disguised as tourists were intercepted on Oct. 16 and 17 before they could board their flights to Hongkong and Macau en route to their final destinations in the United Arab Emirates and Qatar.

“It appears that these victims were recruited by a syndicate which separately booked them on several flights in order to mislead our immigration officers on the purpose of their trip. And that is to work abroad without documentation,” Medina said. He surmised that due to the Bureau of Immigration’s intensified campaign against trafficking, the syndicates are attempting to shift their operations to other exit ports. “They are, however, wrong in assuming that we are less vigilant in guarding our ports outside Manila,” Medina stressed, adding that Bureau of Immigration Commissioner Jaime Morente has already alerted Bureau of Immigration personnel manning the different reports to be extra vigilant following the incident at Mactan airport. Ma. Asuncion Palma-Gil, BI-MCIA’s travel control and enforcement unit (TCEU) head, disclosed that the 17 passengers were offloaded from five different flights and that 13 of them were hired to work in Dubai while the other four were bound for Qatar. “They all admitted having UAE and Qatari visas in their possession and that they were recruited by individuals they only met via Facebook or through their people they know currently working in Dubai and Doha,” Palma-Gil said. She added that the passengers were not familiar with their travel itinerary and what they would do upon arriving in Hongkong and Macau as they would only rely on instructions to be given to them by their handlers. “This is a clear case of human trafficking wherein victims who are jobless and have no visible means of support are sent abroad and put in harm’s way to satisfy the cravings for profit of unscrupulous recruiters,” Palma-Gil added. The women were later turned over to the MCIA Inter-Agency Council Against Trafficking (IACAT) for assistance and further investigatio.

MANILA, Philippines — Bureau of Immigration agents announced the arrest of a Korean national wanted by authorities in Seoul for swindling two compatriots of more than 100 million won. Bureau of Immigration intelligence officer Bobby Raquepo, chief of the bureau’s fugitive search unit (FSU), identified the captured Korean as 49-year-old Kim Sang Wook, who was arrested on Tuesday in Malvar Street, Malate, Manila.

The FSU operatives were armed with a mission order from Bureau of Immigration Commissioner Jaime Morente who issued the same at the request of the Korean embassy in Manila. Raquepo said that a Korean court had issued an arrest warrant against Kim who is also the subject of a red notice from Interpol. He further disclosed that Kim was a teacher wanted for swindling his victims of 100 million won. Kim was reportedly found defrauding his victims six times by deceiving and promising his victims’ sons that they could be employed as teachers. Investigations revealed that Kim fled to Manila on February 2015 under a tourist visa and has not left the country since then. According to Raquepo, Kim will be deported, considering he is an undesirable alien for being a fugitive from justice. Morente then warned foreign fraudsters and fugitives staying in the country, “We are serious in our drive against undesirable aliens. We will hunt each one down, deport them, and bar them from re-entering the country,” he said. Kim is temporarily detained at the Bureau of Immigration Warden Facility (BIWF) pending deportation proceedings. Meanwhile, Morente lauded the improvements made at the BIWF. BIWF Chief Remiecar Caguiron detailed that the BIWF has undergone a total revamp under her administration. "Apart from improving the quality of our personnel through seminars on jail management and human rights, we have made numerous improvements in the facilities of the detention center," said Caguiron. "We renovated a lot of areas that were already old and dilapidated, and used the open spaces to create a multi-purpose hall," she shared. Caguiron clarified that the BIWF "is not a jail, but a detention center" where erring foreigners are temporarily held pending their deportation. The new facilities were made following calls to improve the conditions of the detention center. "The BIWF has a capacity of less than 200, hence it is quite a challenge especially since our intel has been conducting numerous arrests. The management actually is looking at the possibility of setting up another bigger detention center somewhere north of Luzon. This is part of our long term plans," she shared.

Bureau of Immigration (BI) Port Operations Division (POD) Chief Grifton Medina said 31-year-old Zare Bahari Bahareh was immediately excluded hours after she arrived at the NAIA terminal 3 aboard a Cebu Pacific flight from Dubai.

Medina said Zare Bahari, who was a respondent to an assault and battery case in Dagupan, was intercepted by Bureau of Immigration men upon seeing that her name has a hit in their derogatory database. According to Bureau of Immigration NAIA Terminal 3 Head Bradford Allen So, Zare Bahari was excluded as she is the subject of a red notice from Interpol. So likewise disclosed that during the implementation of Zare Bahari’s exclusion, another Iranian national they later identified as Minbashi Moeini Morteza had then approached the Immigration arrival area. "Morteza was unruly. He was able to enter restricted areas at the airport, collect the luggage of Bahari, and sneak into the immigration arrival area to personally fetch his compatriot, surprisingly without an airport pass," So said. He added that Morteza's presence in the arrival area was a brazen case of security breach that they had to immediately report to the Manila International Airport Authority (MIAA). "Morteza and Zare Bahari were already causing a scene, and had to be forced apart by airport authorities, as Zare Bahari was screaming ‘Jesus kill you all Filipinos!’” So further shared. Morteza was reportedly then taken into custody by airport security personnel. Immigration Commissioner Jaime Morente then expressed his dismay towards the incident. “Foreign nationals should respect our laws when they are in our country. Improper behavior and derogatory remarks gave the officer further reason to deny Zare Bahari’s entry,” he stated. He likewise requested airport authorities to conduct an investigation on the illegal entry of Morteza in the airport premises. “This is a clear security breach, and should not be taken lightly. We are deeply concerned as to how this person was able to enter the airport without inspection,” he added.

They all eventually admitted having United Arab Emirates (UAE) visas in their possession and that their final destination is Dubai where they were recruited to work as household service workers.". Four undocumented overseas Filipino workers (OFWs) hired to work as household service workers (HSW) in Dubai were stopped at the Ninoy Aquino International Airport (NAIA) for attempting to leave for Bangkok in the guise of being tourists, the Bureau of Immigration (BI) said.

“We are in the midst of an intensified campaign against human trafficking, thus we are warning aspiring OFWs to comply with the rules and requirements for overseas workers,” said Bureau of Immigration Commissioner Jaime Morente in a statement. “We are likewise warning recruiters and other cohorts not to attempt to assist or vouch for illegal workers, as you will face human trafficking cases which could lead to life imprisonment,” he added. The Bureau of Immigration chief issued the statement after immigration officers intercepted four women at the NAIA Terminal 3 who pretended they were going to Thailand as vacationing tourists. Bureau of Immigration port operations division chief Grifton Medina reported that the women were about to board a flight to Bangkok last Oct. 9 when they were intercepted by members of the bureau’s travel control and enforcement unit (TCEU). “They pretended that they all knew each other as they were co-workers and friends,” said Medina. He added that a woman who allegedly recruited the workers and was supposed to vouch for them and accompany them in their trip was also barred from leaving. They were all turned over to the Inter-Agency Council Against Trafficking (IACAT) for investigation and filing of appropriate charges, Medina said. Bureau of Immigration TCEU chief Timotea Barizo said the women initially claimed to be employed at the travel agency owned by their recruiter, who also arranged their trip to Bangkok. “During immigration inspection, the recruiter would act as the spokesperson for the entire group. She kept on answering for everyone, and has everyone’s documentation. This caused our officers to further doubt their story, hence they verified and eventually discovered the deceit,” said Barizo. “We are noticing here a pattern where trafficking victims are being made to appear as employees of their recruiters. But such scheme will not work anymore, luma na yan, we knew of this trick a long time ago,” Barizo said. The names of the passengers were not divulged to a prohibition in the anti-trafficking law.

Seven suspected victims of human traffickings were rescued by immigration officers at the Ninoy Aquino International Airport (NAIA) after they were intercepted for possessing spurious travel documents. “These fraud syndicates continue to ignore our warnings. There will be no letup in our campaign against human trafficking and their victims will not be allowed to leave if they are caught,” said Bureau of Immigration (BI) Commissioner Jaime Morente. Morente issued the statement after the seven passengers were stopped from leaving the country last week in three separate instances at Terminals 3 and 1 of the NAIA.

The latest interception involved five passengers who pretended to be tourists in attempting to leave for Malaysia last Oct. 5 at NAIA 3. “They admitted paying their recruiters fees ranging from P125,000 to P175,000 each in exchange for processing their jobs and travel papers to Australia, which was their final destination,” said Bureau of Immigration Port Operations Division Chief Grifton Medina. The passengers were reportedly accompanied by a woman who was also stopped due to a pending court cases of estafa and illegal recruitment. ‘They also presented fraudulently manufactured employee IDs which were given to them by their handlers to make it appear that they are gainfully employed here and are thus legitimate tourists,” Medina added. Earlier, the BI’s travel control and enforcement unit (TCEU) reported that a Malaysia-bound woman was intercepted also at NAIA 3 last Sept. 24 for having a spurious United Kingdom visa and tampered pages on her passport. “We also discovered that she was already previously barred from leaving the country on suspicion of being a tourist worker,” Bureau of Immigration-TCEU chief Timotea Barizo said. On Oct. 2, TCEU members also stopped from leaving an underage overseas Filipino worker who misrepresented her age by falsifying her date of birth. All seven passengers were turned over to the Inter-Agency Council Against Trafficking (IACAT) for assistance and further investigation.

Bureau of Immigration (BI) officers at the Ninoy Aquino International Airport (NAIA) recently barred from entering the country a Taiwanese fugitive wanted in his country for business fraud. In a report to Immigration Commissioner Jaime Morente, Bureau of Immigration port operations division chief Grifton Medina identified the Taiwanese as 70-year-old Miao Jwu-Yi, who was intercepted September 30 at the NAIA 1 terminal upon his arrival from Hongkong. Medina said Miao was apprehended after his name registered a hit in the Bureau of Immigration’s database of blacklisted foreigners with derogatory records.

He was flown to Taipei the following day, together with his escorts from the Taipei Economic and Cultural Office (TECO) in Manila, Medina added. TECO officials said Miao was convicted and sentenced to more than four years in prison in 2017 for violating Taiwan’s Business Entity Accounting Act. The said act prescribes the code of conduct and rules of practice for Taiwan’s accountants and provides penalties for violators. Miao reportedly went into hiding and fled Taiwan upon learning of his conviction, prompting the district prosecutor’s office in Taipei to issue a warrant for his arrest. Atty. Rommel Tacorda, the BI’s border control and intelligence unit (BCIU) chief, disclosed that prior to his flight, Miao was issued a single-use travel document by TECO because the Taiwanese government had earlier cancelled his passport. “He remains in our blacklist, thus he will be barred again from entering our country if he attempts to return in the future,” Tacorda said.

Bureau of Immigration (BI) Commissioner Jaime Morente identified the alien as 71-year-old Katsumi Ohno, who was arrested last Sept. 30 along the service road in Roxas Blvd., Pasay City by members of the bureau’s fugitive search unit (FSU). Morente said the arresting agents were armed with a warrant of deportation which he signed pursuant to a deportation order that the Bureau of Immigration board of commissioners issued against the Japanese on June 20, 2019.

Ohno’s deportation was reportedly sought by the Japanese embassy last February when it informed the Bureau of Immigration about a standing arrest warrant for fraud that a Japanese court issued against him. “We also learned that his passport already expired in November last year, making him an undocumented alien,” Morente said. He added that Ohno has been included in the immigration blacklist to prevent him from returning to the Philippines. Investigation also revealed that Ohno has been hiding in the country for nine years or since April 10, 2011 when he arrived as a tourist via a flight from Hongkong. Ohno is currently detained at the Bureau of Immigration jail in Bicutan, Taguig City.

The Bureau of Immigration (BI) announced the deployment of 67 newly-hired immigration officers to the country's international airports after graduating on Wednesday, October 2. According to Bureau of Immigration Commissioner Jaime Morente, this is part of the agency's continuous efforts to improve its services to the travelling public. "The graduation of the officers is timed prior to the projected influx of passenger in the airport in the next few months for the upcoming South East Asian Games, and at the same time as the holiday rush begins," Morente said.

Morente said that the new recruits underwent the Border Control Officers Module (BCOM), a six-month long training designed to teach immigration laws, rules, and procedures. "The 67 graduates, which consist of 38 females and 29 males, were divided into two classes. They began training at the Philippine Immigration Academy in Clark Pampanga last April. After three months, they were assigned at the airports and the Bureau of Immigration's main office for another three months as on-the-job trainees," Morente said. Morente further shared that aside from the usual lectures, it is the first time that they have included Disaster and Emergency Preparedness Response seminar in the program. "This kind of training adheres with the changing times. We want our officers to be fully prepared in the field even during emergency situations," he said. The Bureau's Port Operations Division (POD) Chief Grifton Medina likewise disclosed that as part of their continuous effort to address the problem of congestion in our airports, all graduates have signed a lock-in contract with the POD stating that they will be deployed in our ports for a minimum of two years. "We want the concept of genuine public service to be inculcated in our front-liners. We need them to be committed mentally and physically to the job," Medina added.

The Bureau of Immigration (BI) urged aspiring Overseas Filipino Workers (OFWs) to avoid human traffickers and illegal recruiters who would prey on their vulnerabilities, after the agency uncovered the recurrence a modus operandi of a syndicate that houses and trains its victims at safe houses before deployment abroad. Bureau of Immigration Commissioner Jaime Morente issued the reminder after two women were recently barred from leaving the country for misrepresenting their age and narrating their experience with the syndicate that recruited them. The women had alleged that they were kept by their handlers in a safehouse in Paco, Manila for two months before they were booked for their flights.

Morente instructed the bureau’s port operations division (POD) and travel control and enforcement unit (TCEU) to conduct strict profiling and inspection of departing passengers to ensure that no underage OFWs are able to leave. According to Bureau of Immigration POD chief Grifton Medina, the two women, aged 19 and 20, were intercepted last Sept. 21 at the NAIA terminal 2 in their attempt to board a connecting flight from Dubai to Saudi Arabia. Medina said the passengers both presented valid passports, visas, job contract, and overseas employment certificates but the birth dates in their documents were intentionally altered to make it appear that they meet the age requirement for Household Service Workers which is 23 years. “Both women initially claimed that they were 26 years old, but eventually admitted their real age upon questioning,” Medina said. Bureau of Immigration-TCEU chief Timotea Barizo said that the women recounted how they were housed for two months at a safehouse in Manila where they were briefed and taught by their recruiters how to respond to questions from immigration officers. “We've heard this in the past, usually victims would be briefed a few days before their flight. But now they're actually housed for months to train on how to evade immigration questioning," said Barizo. "The two victims admitted that their documents were given only prior to departure, and that they were told to open it only after check in. This forces them to go on and comply with the scheme despite the discrepancy since they are already there,” Barizo said. Morente reiterated his reminder to OFWs not to fall prey to these schemes. "Transact only with legitimate agencies accredited by the POEA (Philippine Overseas Employment Administration)," he reminded.

Bureau of Immigration Commissioner Jaime Morente disclosed that 71-year-old Alfons Hermann Keittl was arrested last Thursday in Ozamis City, Misamis Occidental by operatives from the bureau’s fugitive search unit (FSU). Morente said Keittl’s deportation was sought by the German embassy in Manila which informed the Bureau of Immigration about an arrest warrant that a local court in the town of Landshut, Germany issued against the fugitive last July.

The Bureau of Immigration (BI) urged aspiring Overseas Filipino Workers (OFWs) to avoid human traffickers and illegal recruiters who would prey on their vulnerabilities, after the agency uncovered the recurrence a modus operandi of a syndicate that houses and trains its victims at safe houses before deployment abroad. Bureau of Immigration (BI) officers at the Ninoy Aquino International Airport (NAIA) recently barred from leaving the country seven undocumented overseas Filipino workers (OFWs) who attempted to depart in the guise of being tourists. BI Port Operations Acting Chief, Grifton Medina said the passengers were deferred to depart after being intercepted on separate occasions last week at NAIA Terminal 2 and 3.

“They were not allowed to leave after they admitted under questioning that they are not tourists but are actually going abroad to work,” He said. He reiterated his warning to those who intend to work overseas not to evade the travel requirements for OFWs by pretending to be tourists as “there is very little chance you will not be caught by our vigilant immigration officers at the airports.” Reports submitted by the Travel Control and Enforcememt Unit (TCEU) Head, Ma. Timotea C. Barizo to BI Commissioner Jaime Morente disclosed that two of the passengers were intercepted at NAIA 2 last Sept. 14 as they were about to depart for Hongkong. They were barred to depart after they were noticed traveling with a woman who is in the bureau’s alert list of suspected human traffickers facilitating the deployment of undocumented workers abroad. On Sept. 15, four Malta-bound “tourist workers” were apprehended at the NAIA 2 departure area after they presented questionable documents regarding the purpose of their travel. They initially claimed they were attending a symposium but later admitted during interview that they were hired to work as maids in Malta. They further alleged that each of them gave P500,000 to their handler in return for processing their travel documents and visas. Barizo also reported the interception last Sept. 17 of a Filipina bound for Istanbul, Turkey who was caught in possession of a fraudulent overseas employment certificate. All seven passengers were turned over to the NAIA Task Force Against Trafficking in Person (NAIATFAT) for further investigation.

PASAY, Philippines--Bureau of Immigration (BI) officers at the Ninoy Aquino International Airport (NAIA) have intercepted 16 undocumented overseas Filipino workers who attempted to leave the country by disguising themselves as tourists. In a report to Immigration Commissioner Jaime Morente, BI port operations division chief Grifton Medina said the passengers were intercepted in two batches at the NAIA 3 terminal last Aug. 7 as they were about to board their Cebu Pacific Airways flights to Taiwan and Hongkong. One batch consisted 12 of the passengers who were bound for Taipei while the other group of four passengers were bound for Hongkong. “They all confessed during questioning that they are not tourists and that they were hired to work abroad by illegal recruiters,” Medina said. Information obtained from BI-NAIA 3 travel control and enforcement unit (TCEU) chief Anthony Lopez revealed that the 12 Taipei-bound and mostly women passengers admitted that their final destination is the United Arab Emirates where they were recruited to work by a travel agency based in Manila. On the other hand, the four Hongkong-bound women confessed that they were going to Hongkong to process their visas to China where they were allegedly promised jobs by a FilipinoChinese businessman. All 16 passengers were turned over to the Inter-Agency Council Against Trafficking (IACAT) for investigation "These syndicates just won't stop," said Morente. "Many still attempt to leave in the guise of being tourists, some in fake group tours. We wish to warn our kababayan about this modus, do not fall for the sweet-talk of these illegal recruiters. They only want to profit at your expense," he warned.

The Bureau of Immigration (BI) deported a Korean fugitive wanted by authorities in his country for operating an illegal gambling website.

According to his report to Bureau of Immigration Commissioner Jaime Morente, Bureau of Immigration Port Operations Chief Grifton Medina identified the fugitive as 35-year-old Jung Hyeongwook, who left the country last Sept. 3 aboard a Philippine Airlines flight to Pusan. He further said Jung was deported a few days after he surrendered to the Korean embassy in Manila which earlier cancelled his passport. The passenger was reportedly issued a temporary travel document by the embassy to facilitate his departure and was later arrested upon his arrival in Korea. 

PASAY CITY - Immigration authorities at the Ninoy Aquino International Airport (NAIA) have intercepted again two South Koreans who are wanted fugitives from justice in their country. In a report reaching Bureau of Immigration Commissioner Jaime H. Morente that Koreans Hong Chan Woo, 24 years old, and Jun Juman, 36 years old, whose names are both in the Interpol’s wanted list, were intercepted on Aug. 19 and 20, respectively, at the departure area of the NAIA 3 terminal.

What are the advances in hemorrhoids treatment?

 

Due to this reputation of being a painful operation, advances in hemorrhoid surgery have focused on making it less painful.  One such operation is stapled hemorrhoidectomy. Using a specially-designed, single-use stapler instrument (which was modified from the staplers used to reconnect intestines during major abdominal surgery), the hemorrhoids are excised and the anus repaired from the inside, something which is difficult to do with traditional surgery.  The end result is that the surgical wound is inside, and no wound is visible outside the anus. Scientific studies have proven that this is less painful than traditional surgery, although with a small recurrence rate.

Another advanced surgical technique is Transanal Hemorrhoidal Dearterialization (THD).  Using a doppler ultrasound, the blood vessel supplying the hemorrhoid is identified and ligated with stitches.  Deprived of its blood supply, the hemorrhoid then shrinks and shrivels.   Again there is no wound outside and is therefore almost painless.  However, because it is a new technique, not very many studies evaluating its long-term effectiveness have come out.

 All surgical procedures for hemorrhoids, whether traditional or innovative, may sometimes develop complications such as bleeding, infection and stricture. It is important therefore when seeking treatment, to see an expert in anal diseases.

The term hemorrhoids are associated with the symptoms when this part of the body becomes enlarged, inflamed, thrombosed or prolapsed.

Many people have them without suffering from any symptoms, and therefore these may be safely left alone. Treatment is usually required for those whose hemorrhoidal symptoms such as excruciating throbbing pain affect quality of life and daily living. There are many traditional remedies and non-surgical treatments for hemorrhoids. However, if none of these work, surgical procedures may be required as a last resort.

 

How do hemorrhoids look and feel? 

Hemorrhoidal cushions are skin, vascular and soft tissues that line the anal canal, playing a small role in defecation and continence.  Everyone has them, although with our daily travails of straining and movement, they do change over time, becoming more lax or loose as we age. 

Hemorrhoidal disease occurs when these changes result in bothersome symptoms such as bleeding, prolapsed, or pain.  Almost everyone may develop symptoms at some point in their life, but these are not life-threatening, and may be safely observed. Treatment is usually required for those whose hemorrhoidal symptoms impact on quality of life and daily living.

 

What are the symptoms of hemorrhoids? 

Bleeding from hemorrhoids is usually bright red, often dripping into the toilet bowl, or noticed when wiping. The stools themselves are normal looking, indicating that the bleeding is originating only from the anorectal area.  Sludge of dark blood and stool mixed thoroughly together is usually from a source within the small and large intestines and not hemorrhoids. Hemorrhoidal bleeding is painless, and this differentiates it from anal fissures, which is also a very common anal condition that presents with similar bright red bleeding during defecation, but with anal pain.

Hemorrhoids only become painful when they thrombosed or incarcerated.  Thrombosed hemorrhoids occur when blood clots within the hemorrhoids, causing very obvious swelling and pain.  While surgery for this may be effective during the first 4 days of the attack, if left alone the swelling and pain gradually subside and disappear after 2 weeks.  Usually this occurs just once or twice in a person’s lifetime, and it is uncommon to have repeated episodes of thrombosis. 

Doctors often classify patients as to having either external or internal hemorrhoids.  External hemorrhoids are located just outside the anus, and hence are always visible on routine inspection.  Many people have them without suffering from any symptoms, and therefore these may be safely left alone.  Sometimes external hemorrhoids may make cleaning difficult, allowing sweat and anal discharge to hide between folds, resulting in anal itching.

Internal hemorrhoids arise from inside the anus and are therefore only visible when, because of excessive laxity, they protrude outside.  This condition is called prolapsed, and usually occurs during defecation.  When internal hemorrhoids prolapsed they may either slip back inside on their own, or require manipulation by the person for it goes back in.  When the prolapsed hemorrhoids cannot be returned, they become trapped outside, resulting in pain and swelling.  This scenario is called incarcerated hemorrhoids.

 

What are the forms of treatment for hemorrhoids– surgical and non-surgical? 

Hemorrhoids only require treatment when there are symptoms.  And the type of treatment depends on the severity of symptoms.  Mild symptoms may be managed just by eating a lot of fiber and taking in bulk laxatives such as psyllium to improve the consistency of stools and ease bowel movement.  Some over-the-counter creams may provide symptomatic relief.

Non-surgical and almost painless treatments for hemorrhoids include injection sclerotherapy, infrared photocoagulation (often mislabeled as “laser treatment”), and rubber band ligation, any of which can be done safely in the clinic as an out-patient procedure.  In scientific studies comparing these methods, rubber band ligation was found to be the most effective and cheap, which is why it is the most common non-surgical procedure for hemorrhoids in the world today.

Non-surgical treatment is often ineffective for large, incarcerated or thrombosed hemorrhoids.  Surgery is usually recommended in these situations, and also when non-surgical treatment has failed to relieve symptoms.  Traditional surgery requires cutting out the hemorrhoid using a variety of instruments such as scissors, knives, cautery, and ultrasonic dissectors. Unfortunately, any wound created in the outer skin of the anus results in often Sever post-operative pain, which is responsible for the reputation of hemorrhoid surgery being one of the most painful of operations.
